Monitor platelets to ensure they’re in appropriate range

PTT
What labs are monitored with Warfarin (Coumadin)? How often must the labs be monitored?
PT and INR
Labs must be checked every 3-4 weeks
What labs are monitored with Apixaban (Eliquis) and Rivaroxaban (Xarelto)?
No labs are monitored for effectiveness
What education should be provided when a patient starts an anticoagulant?
They will bruise more easily
If they start to bleed it won’t stop as easily
Educate on s/s of bleeding
What is the antidote for Heparin?
Protamine Sulfate
What is the antidote for Warfarin (Coumadin)?
Vitamin K
