What are some tocolytic meds?

When do you use tocolytic meds?
Before 34 wks
Dilation more than 4 cm
When is tocolysis contraindicated?
Placental abruption Chorioamnionitis IUFD Lethal fetal anomaly Severe IUGR with reversed Doppler flow Severe pre-eclampsia (HELP, eclampsia) >34 weeks gestation
Nifedipine ADR
Mom- transient hypotension, flushing, HA, dizzy, N
Baby- none
Nifedipine contraindications
Cardiac or liver dz, maternal hypotension < 90/50, avoid concomitant use with mag
Terbutaline adr
Mom- cardiac or cardiopulmonary arrythmia, pulmonary edema, myocardial ischemia, hypotension, tachycardia, May worsen glycemic control
Baby- fetal tachycardia, hyper-insulinemia, hyperglycemia, myocardial and septal hypertrophy, myocardial ischemia
When is terbutaline contraindicated?
Cardiac arrhythmia
Magnesium sulfate adr
Mom- flushing, HA, muscle weakness, diplopia, dry mouth, pulmonary edema, cardiac arrest
Baby- lethargy, hypotonia, respiratory depression, demineralization with prolonged use
Mag contraindicated
Myasthenia gravis, renal disease
Mag clinical pearl
If pt 23-32 weeks pregnant and delivery is expected in next 24 hours, give mag for 12 hours for protection against cerebral palsy
