types of mastitis?
infection and congestive
what causes infective mastitis?
Staph Aureus = Most Common
Strep + Candida
when is infective mastitis most commonly seen?
mostly in lactating women secondary to nipple trauma → Especially in primigravida
what is congestive mastitis?
Bilateral breast enlargement 2-3 days postpartum
s/sx of infective mastitis?
Unilateral breast pain → Especially one quadrant → With tenderness, warmth, swelling & nipple discharge
s/sx of congestive mastitis?
Bilateral breast pain & swelling → May have low grade fever & axillary lymphadenopathy
s/sx of a breast abscess?
Induration with fluctuance (due to pus) → Rare
tx of infective mastitis?
Supportive measures (warm compresses, breast pump) + Anti-Staphylococcal Abx
Dicloxacillin, Nafcillin, Cephalosporin
Fluconazole if fungal
pt ed for infective mastitis
Mothers may continue to nurse or use breast pump
tx for congestive mastitis? breastfeeding not desired
If women does not want to breastfeed → Ice packs, tight-fitting bras, analgesics, avoid breast stimulation
tx for congestive mastitis? if breastfeeding desired
If breastfeeding desired → Manually empty breast completely after baby is done breastfeeding, local heat, analgesics, continue nursing
tx of breast abscess
I&D → Discontinue breastfeeding from the affected breast
fribrocystic breast disorder PP?
Fluid-filled breast cyst due to exaggerated response to hormones
what is the MC breast disorder in 30-50 yo
fibrocystic
s/sx of fibrocystic breast disorder
Usually multiple, mobile, well demarcated lumps in breast tissue
Often TENDER, bilateral
Usually no axillary involvement nor nipple discharge
Breast cysts may INCREASE OR DECREASE IN SIZE W/MENSTRUAL HORMONAL CHANGES

tx of fibrocystic breast disorder
Most spontaneously resolve
+ FNA removal of fluid if symptomatic
fibroadenoma of the breast PP
Composed of glandular & fibrous tissue → Collagen arranged in ‘swirls
when are fibroadenomas most oftenly found?
late teens-early 20s
s/sx of fibroadenomas?
Smooth, well-circumscribed, nontender, freely mobile, rubbery lump in the breast
Gradually grows over time & DOES NOT USUALLY WAX & WANE WITH MENSTRUATION
May enlarge in pregnancy
No axillary involvement or nipple discharge
tx of fibroadenomas
Observation → Most small tumors resorb with time
+ Excision → Not usually done
