Escalating Medical Strategy in Status Epilepticus

A

1st- Lorazepam/ Ativan, 2- 4 mg

**Benzo may be repeated before moving on to the next line

2nd- Levetiracetam/ Keppra, 2-3 g

3rd- Phenobarbital, 15-20 mg/kg

A

Cardiovascular collapse due to rapid infusion of phenytoin

hypotension, bradycardia, cardiac toxicity (due to propylene glycol diluent)

HTN emergency Acute Pulmonary Edema Rx

A

Nitroglycerin: 0.4 mg q5min sublingual PRN
or
Nitroglycerin IV

Bonus:
Enalaprilat: 0.625-1.25 mg IV bolus or
Captopril: 25-50 mg PO/sublingual or
Nitroprusside: Initial dose 0.3 µg/kg/min IV titrate to effect by 0.5 µg/kg/min q5 min (max 10 µg/kg/min)
