nonbacterial thrombotic endocarditis
epidemiology
common
occurs in 75% pts w/ malignant tumors (esp. mucinous adenocarcinomas)
common in pts w/ DIC, chronic sepsis, and Swan-Ganz right heart catheterization
Pathology
small (1-5mm) fibrin and platelet thrombi
most commonly on atrial side of mitral valve, usually on line of valve closure
second most common site is on ventricular side of aortic valve
incidence of infection marantic endocarditis and rheumatic disease on valve correlates w/…
resting pressure on closed valve:
- -mitral=116mmHg
- -Aortic=72mmHg
- -Tricuspid=24mmHg
- -Pulmonic=5mmHg
hard work (on valves) leads to…
injury, inflammation, clot deposition, and infection
complications
systemic emboli (in about half of cases) to:
- -kidneys
- -spleen
- -brain
- -gut
- -heart
infection
(converts it to infective endocarditis)
Important b/c…
- it is common
- frequently embolizes
- precursor to infective endocarditis
