Sit resident up from a lying position on the side of the bed (dangling)
Beginning tasks (gate belt with you)

Completion Tasks
- Position the resident comfortable and safe
Rail up
Lower bed
Call light - remove or discard gloves, protective equipment
- wash hands
*check safety/resident environment call light
*open curtains.
*care for the equipment
*document actions/observations
Ice Bag
- Provide privacy for resident
- explain the procedure and answer questions
*First fill ice bag with water to check for holes then empty - Fill 1/3 full of ice, remove excess air, seal bag and dry outside
wrap a protective cover around bag
Place a waterproff pad under the body part - place wash cloth on body area then apply ice bag to body part
*check every 5min for cyanosis or complications
removed after 15-20min, dry the area
Perform completion tasks
Apply Antiembolism stocking (TED HOSE)
- Provide privacy for resident
- explain the procedure and answer questions
Expose the leg
placed hand inside the stocking and grasp the heel of stocking and fold back over hand
Place stocking over toes
slide stocking up the leg, seam on inside of thigh
make sure not twisted and no wrinkles
monitor skin changes, color, temp, tingling,numbness
perform completion tasks
Demonstrate how to collect mid stream /clean catch urine specimen
- Provide privacy for resident
- explain the procedure and answer questions
Put on gloves
prepare label
get up ready with holder top
spread labia with thumb and index finger
use antiseptic solution to cleanse urethral area front to back
ask resident to void into toilet then collect urine mid stream
cap top of container, add label
put container in biohazard bag and refridgerate
remove gloves
wash hands
make sure resident is safe
perform completion tasks
Measure Urine in graduate
Wash hands
* assemble equipment (2wipes, paper towel, graduate, gloves)
* Provide privacy for resident
* explain the procedure and answer questions
know amounts ml/cc 1oz=30ml/cc
Bed pan
Wash hands
* assemble equipment
2 clean towels, bath blanket, bed pan, 2 pair of gloves, chux, bag for dirty,
* introduce self and verify resident identity (wrist band, name & DOB)
* Provide privacy for resident
* explain the procedure and answer questions
-raise bed to working height, lower bedrail, lower head of the bed
-ask resident to bend knees and raise buttocks, or turn to side and press pan down into bed when turning resident back on top
-make sure bedpan centered
-raise head of bed in sitting semi-fowlers to fowlers position where comfortable
-lower bed and give call light
-give privacy 5min check
-new gloves
-lower head of bed, raise to working height
-safely stabilize bedpan when turning and removing
-do perineal care to clean resident, privacy
-new chux if necessary
-remove gloves
-lower bed and, rails, give call light
-new gloves
-note color, amount, smell , consistency of stool
-flush contents of bedpan
-clean and disinfect bedpan, dry
perform completion tasks
Transfer resident with right sided paralysis from bed to wheelchair using transfer belt
-Beginning tasks, explain what you are going to do, have gate belt with you
-position wheelchair on left side of bed and the top of the bed, lock wheelchair brakes
-put on shoes
-lock arm technique to sit up
-pivot, hold resident the whole time, ask if dizzy
-wrap belt around them, should be snug with buckle off center
-tell patient to place weak arm across there body, hold your shoulder with left
-make sure feet are placed on floor, block them with your knees
-explain what happens next, stand at the count of 3, under hand under belt
-scoot with your left foot as they move to wheelchair
-ask if they feel the chair behind them, block their knees again
-ask them to reach back to wheelchair and slowly help them sit
Reposition resident in wheelchair
Tell resident when you are going to do, reposition them in the chair
Make sure it is locked
Bend knees in front so them can help left
reach under my right on their left, my left hand on their right arm
count to 3 and left to reposition them
place feet on footrest if applicable
Take and record blood pressure
Tell resident you are going to take their blood pressure
-clean ear with alcohol swab, and diaphragm end, make sure on
-make sure arm is resting on table relaxed palm up
-look for brachial pulse up pinky side of arm
-wrap cuff on arm where pulse meets line
-position stethoscope over brachial artery
-inflate about normal systolic, 160
-deflate cuff in timely manner
mark systolic 100-140
and diastolic 60-90 measurements
Respiration
Begin counting when the residents rose
Count for 30secs and multiply by 2
student says start and end count
record measured respiration rate
verbalize normal respiration range (braidnea below 12) 12-20 (tachynea if about 20)
Pulse
locate Radial pulse with fingers (thumb size of wrist)
determine rhythm and decide whether to count for 30 or 60
count pulse for 30 multiply by 2
student must say start and end
verbalize the range of normal pulse (braidacardia too low) 60-100 (tachyncardia too high)
Temperature
Activate thermometer
shake down class to lowest if necessary
cover thermometer with plastic sheath
place thermometer under tongue
instruct resident to close mouth
remove after 2 to 3min
read and dispose of cover appropriately
verbalize normal temp
oral
rectal
axillary
tympanic 98.6 +-
Transfer belt and assist patient with ambulation using transfer belt using walker
