What are some of the main side effects of antipsychotics?
Akathisia Weight gain QT prolongation Prolactin elevation Sedation
What receptors are responsible for the following side effects:
- Hypotension
- Dystonia
- Akathisia
- Hyperprolactinemia
- Sedation
- Weight gain
- Anticholinergic (dry mouth, constipation, blurred vision, cognitive)
- Hypotension: Alpha-1
- Dystonia, Akathisia, Prolactin: Dopamine-2
- Sedation and weight gain: Histamine-1
- Anticholinergic: Muscarinic 1-5
What is Neuroleptic Malignant Syndrome? When is the onset?
Tetras of Fever, Rigidity, mental status change and autonomic instability (tachycardia, diaphoresis)
Onset usually within 2 weeks of starting antipsychotic
What are some things to consider prescribing with Clozapine when initiated?
Stool softeners due to constipation risk
What are three general antipsychotic-induced motor side effects to look for?
Akathisia
Parkonsinism
Traduce Dyskinesia
What are some possible treatments for Akathisia?
- Reduce dose or switch
- Propranolol
- Mirtazapine
How often should AIMS be completed?
At least annually
Management of TD?
- Slowly taper
- Switch to Seroquel or Clozapine if possible
- VMAT inhibitors for symptoms
Management of Parkonsinism?
Lower dose, switch, anticholinergic
What is something you can add to reduce prolactin levels?
Abilify
