Who is responsible for preserving evidence after a serious accident?
Anyone involved, including the signaller.
What are the emergency actions that must be prioritized over preserving evidence?
- Secure the train
- Protect the line
- Get the electric traction current switched off
- Call the emergency services
- Ensure safety of the public and personnel.
Under what circumstances can evidence of a serious accident be disturbed?
When instructed by a rail incident officer (RIO), police incident officer, rail accident investigation branch (RAIB) investigator, or other senior investigator.
What should be noted immediately after a serious accident?
Evidence that might be lost over time, such as wheel-tyre and brake-block temperatures, and brake-gauge readings.
After a serious accident when is it permissible for anyone involved to enter a driving cab?
- To secure the train
- To carry out rescue operations
- To use the cab radio to call emergency services or speak to the signaller
- To get equipment for protection or extinguishing a fire
- To note short-life evidence.
What must you do after entering a driving cab for emergency actions?
- Make a detailed note of the original position of equipment
- Provide this information to an investigating officer before leaving.
What actions must be taken regarding cab doors after an accident?
- Lock the doors to the cab
- Ensure other driving cab doors are locked.
What equipment must not be moved after an accident?
Signalling equipment directly associated with the accident.
What must a driver do if any vehicle has been derailed?
Do not allow it to enter or continue in service until examined by a rolling stock technician.
However, if the derailment was at slow speed, vehicles that were not derailed or coupled next to a derailed vehicle can be examined at the first suitable location.
What should a driver do after a collision with an obstruction on the line?
- Bring the train to a stand
- Check for damage that might have been caused by the collision & might affect its safe movement.
- Speak to the train operating company’s control if it is possible that any of the following have been damaged;
- Brake equipment.
*Coupling and drawgear. - Vehicle suspension.
- Wheelsets.
- Current collection equipment.
- Lifeguards.
What must a signaller do after receiving a report of a body on the line?
- Find out the location of the body
- If necessary arrange for emergency switch-off of electric traction current
- Inform Operations Control.
The signaller can allow train to proceed if they - Have been told that the body is clear of the line
- The body cannot be struck by a passing train
- Body parts are not recognisable
(Recognisable body parts can be passed if not in view of passengers with drivers agreement)
What should a driver report regarding flood water, stopping specially to do so if necessary?
- If it is up to the bottom of the rail head
- If it is up to the top of the rail head
- If it is above the top of the rail head
- If it is moving and likely to dislodge ballast
- If it has dislodged ballast.
During flooding when water levels are between the bottom and top of the rail head what is the maximum speed?
- 5 mph (10 km/h)
What must a driver do if they see a build-up of snow affecting train passage?
Report to the signaller. You must state if you believe the snow is deeper than 200 mm (8 inches) above the railhead.
What must signallers do before ploughing starts on electrified lines?
- Notify the electrical control operator
- Switch off electricity on lines with conductor rail.
- Switch off electricity on lines with overhead line equipment if snow depth exceeds 900 mm (3 feet).
What types of damage to structures or earthworks must a driver report immediately?
- Landslips
- Washouts
- Embankment failures
- Retaining wall failures.
During reports extreme weather what instructions could you be given?
- affected lines to be examined
- caution trains through affected area
- blanket speed restriction if necessary - the signaller can this via a GSM-R broadcast message that does not have to be acknowledged.
If any vehicle has suffered a collision or heavy impact, you must not allow it, or any part of the train to what?
Enter or continue in service until it has been examined by a rolling stock technician.
If your train has collided with an object on the line and any part of the vehicle has become loose and cannot be secured or might make contact with the track or line side structures what must you do?
You must arrange for the vehicle to be cleared from the running line at the first suitable location.
Before moving the vehicle, you must:
* get the signaller’s permission
* get authority from a rolling stock technician if you are not sure the movement can be made safely
* if possible, move passengers from the vehicle.
During the movement you must not exceed 10 mph (15 km/h) or 5 mph (10 km/h) over points and crossings.
If you see water:
* Rising up from track or cess
* Unusual amounts pooling next to track or cess
* Flowing down or pooling out of cuttings or embankments
What should you do?
Immediately tell the signaller giving exact location and whether water is displacing any material stopping you train specially to do so if necessary.
