Inguinal inspection
A
- Ask pt to stand w/ back against exam table, legs shoulder-width apart
- Instruct him to lift his gown to his waist
- Ask about any abnormalities

Inguinal palpation
A
- Start at iliac crest & palpate down inguinal ligament medially
- Examine pubic hair for lice, scabies

Penile exam
A
- Visually inspect penis (if uncircumcised, ask pt to retract foreskin)
- Palpate both sides of penis & inspect urethral meatus
- Gently open meatus (observe for erythema or discharge)

Scrotal exam
A
- Inspect scrotal skin for moles, rash
- Move scrotum to side & lift to check perineal area
- Palpate scrotal contents:
Testes, epididymis, spermatic cord

Testicular exam
A
- Palpate each testicle btwn your index & middle finger & thumb
- Consistency is like “hard boiled egg”

A
- Located on top of testicle
- Posterolateral
- May be sensitive to touch

Spermatic cords
A
- Bilateral firm cords
- Feel like “BIC pen”
- Palpate from epididymis to inguinal ring
- Palpate btwn thumb & index

Hernia exam
A
- Move slowly
- Invaginate some scrotal skin when inserting finger into inguinal ring
- Use cross-handed method
- Ask pt to cough or valsalva
- Feel for sudden pressure

Rectal & prostate exam
A
- Ask pt to lean over exam table, rest elbows on table, legs apart & knees slightly bent
OR lateral decubitus position - Inspect perianal area:
Spread cheeks
Visualize opening
Check for hemorrhages, fissures, skin tags, & other lesions - Apply lubricant to finger

Rectal exam
A
- Press tip of finger against anal opening, palm facing downwards
- Slowly insert finger & proceed to full length
- Rotate finger clockwise, then counterclockwise
- Note any nodules, irregularities, or tenderness
