Male Genitourinary Flashcards
older male
Does not experience end of fertility Gradual decrease in testosterone after 30 Decreasing pubic hair, penile size Slower, less intense sexual response Erection takes longer to develop Ejaculation shorter, less forceful
prostatic hypertrophy
Straining Loss of force Sense of residual urine Urgency Frequency Hematuria Nocturia
subjective data
Frequency, urgency, nocturne Dysuria Hesitancy and straining Past genitourinary history Urine color Penis/scrotum including self care Sexual activity and contraceptive use STI contact
objective data
Inspection and palpation
Retract foreskin
Check scrotal swelling/inflammation
Assess urine (color, clarity, pH)
When do adolescents hit puberty?
9- African American
10- White
PSA
normal substance made by the prostate gland. Level increases when cancer develops. Enlarged prostate, age and prostatitis can also cause increase.
TSE
timing, once a month
shower, warm water relaxes scrotal sac
examine, check for changes, report immediately
urine
color discolored cloudy foul-swelling bloody pH 4.5-8.0
testosterone decrease
slower and less intense sexual response
erection takes longer to develop, less full and firm
shorter ejaculation
rapid detumescence, especially after 60 years
withdrawal from sexual activity may be caused by
loss of spouse depression preoccupation with work marital/family conflict side effects of medication heavy use of alcohol lack of privacy economic or emotional stress poor nutrition fatigue
testes at birth
descend before birth
measure 1.5-2 cm long and 1 cm wide
Only a slight increase in size occurs during the prepubertal years.

SMR chart notes
enlargement of the testes and scotum pubic hair growth darkening of scrotal color roughening of scrotal skin increase in penis length and width axillary hair growth
oliguria
diminished quantity, less than 400mL in 24 hours
nocturia causes
Occurs with frequency & urgency in urinary tract disorders
Other sources- Meds (diuretics) Habit Fluid ingestion 3 hours prior to bedtime coffee/alcohol have diuretic effect Heart failure or varicose veins
dysuria
pain or burning with urination
Common with-
Acute cystitis
Prostatitis
urethritis
depressants to sexual activity/desire
Antihypertensives Sedatives Tranquilizers Estrogens Alcohol - decreases sexual response more dramatically in older person
NAVEL
used to check for inguinal hernia
Nerve Artery Vein Empty space Lymphatics
urethritis
(Urethral discharge and dysuria) causes painful, burning urination or pruritus. Meatus edges are reddened, everted and swollen with purulent discharge
renal calculi
renal stones (crystals of calcium oxalate or uric acid), cause abrupt severe flank pain with radiation to the groin or abd, N/V, restlessness, gross or microscopic hematuria
variocele
dilated tortuous veins in the spermatic cord
creatinine
Measures glomerular filtration rate
Normal is 0.7-1.5 mg/dL
When GFR decreases by half, serum creatinine level doubles = decreased kidney function
BUN
Blood Urea Nitrogen
Measures 10 to 20 mg/dL - rises w/decrease in fluid volume or increase in protein intake
Rises with decreased kidney function but is less specific
