DSM–5 definition
a. At least five of the following symptoms are present:
i. Irritability
ii. Anhedonia
iii. Unintentional weight loss or gain
iv. Insomnia or hypersomnia
v. Psychomotor agitation or retardation
vi. Fatigue
vii. Feelings of guilt or worthlessness
viii. Poor concentration
b. The diagnosis is made in the absence of obvious organic causes (e.g., anemia,
hyperthyroidism).
Impact on function
a. School and social dysfunction
b. Inability to participate in leisure, ADL, and IADL activities
c. Cognitive impact: loss of concentration, diminished problem-solving ability, poor coping
d. Slowed or increased psychomotor activity
OT Tx plan
- Provide opportunities for self-expression with a focus on values, beliefs, spirituality, and emotional regulation.
- Provide social skills training.
- Assist clients in creating meaningful and satisfying patterns of occupational engagement.
- For clients with suicidal ideation, create a contract for safety
- Eliminate decision making to increase activity engagement and reduce stress.
- Offer simple, structured, familiar tasks.
- Encourage daily routines.
- Arrange for shorter school and work days with reduced tasks and expectations.
- Invite participation; do not force it.
- Keep conversations short and direct.
- Provide a reasonable, just-right challenge.
- Be alert for signs and talk of suicide.
- Improve self-esteem and increase motivation.
- Provide opportunities for self-expression.
- Provide outlets for different emotions.
- Provide opportunities to focus on values, beliefs, spirituality, and emotional regulation
