Lymphoid Neoplasms Flashcards
Which antigens are used to detect lymphocytes?
CD19, CD20 - B cells
CD3 - T cells
What are the markers of lymphoid immaturity?
Tdt, CD34
CD1A is specific for immature T cells
How can you tell if there is a cancerous B-cell population?
If the population is only producing one type of light chain. Normal populations make both kappa and lambda.
Why does the incidence of B-ALL peak at 3 years old?
The number of normal bone marrow pre-B cells is greatest early in life
Why does the incidence of T-ALL peak in adolescence?
This is when the thymus reaches its maximum size.
Morphology of ALL
Marrow is hypercellular and packed with lymphoblasts
Clinical features of ALL
Abrupt onset; fatigue, bleeding,CNS problems
Philadelphia chromosome has a poor prognosis
Morphology of CLL/SLL
- Lymph nodes, spleen and liver effaced with small lymphocytes.
- Smudge cells (disrupted tumor cells)
What distinguishes Follicular Lymphoma from CLL/SLL?
Follicular Lymphoma is characterized by tons of follicles in the lymph nodes as well as a mutated BCL2 gene
Morphology of Mantle Cell Lymphoma
Mantle zone B cells in high number and express cyclin D1
Clinically characterized by painless lymphadenopathy
Whats the difference among the Burkitt lymphomas?
African - all are infected with EBV
Sporadic - 25% are infected with EBV
Morphology of Burkitt lymphomas
Nuclear remnants of apoptotic cells are phagocytosed creating a stary sky appearance
Morphology of Multiple Myeloma
- Destructive plasma cell tumors of axial skeleton.
- RBCs that stick together on blood smear.
- Bence Jones proteins
What is the main morphological feature of Hodgkins Lymphoma?
Reed-Stenberg cells; Multiple nuclei or multilobed nuclei.
Immunophenotype of Nodular Scelrosis HL
PAX 5, CD 15, CD 30
