Definition of UTI?
A
any infection of urinary tract
cystitis - infection of the bladder

Name 4 risk factors of LUTI
A
- Sexual intercourse
- Female gender (shorter urethra)
- Post menopause ( decreased oestrogen - loss of protection from vaginal flora)
- Catheter
- Diabetes mellitus
- Urinary stasis

Name 4 symptoms of LUTI
A
- Suprapubic pain
- Dysuria - pain/difficulty urinating
- Frequent urination
- Urinary stasis - problems voiding

A
- Fatigue
- Incontinence
- May develop delirium

A
1. Urine dipstick # + leukocytes # + nitrites
- Urine culture (Gold standard)

When should urine culture be sent?
A
- > 65 years of age
2. visible / non visible haematuria

Standard tx for LUTI
A
Trimethoprim / Nitrofunatoin for 3 days

Treatment for symptomatic pregnant women
A
- a urine culture should be sent in all cases
- should be treated with an antibiotic for 7 days
nitrofurantoin (should be avoided near term), amoxicillin or cefalexin

Treatment for asymptomatic bacteriuria in pregnant women
A
- a urine culture should be performed routinely at the first antenatal visit
- immediate antibiotic prescription of either nitrofurantoin (should be avoided near term), amoxicillin or cefalexin.
- This should be a 7-day course
the rationale of treating asymptomatic bacteriuria is the significant risk of progression to acute pyelonephritis - a further urine culture should be sent following completion of treatment as a test of cure
