Lidocaine Class
Antiarrhythmic & Sodium Channel Blocker
Lidocaine Indications
V-Tach, Wide Complex Tachycardia w/ pulse, recurrent & refractory V-Fib/Pulseless V-Tach, Pre-intubation in head injuries & pre-treatment of localized pain. Successful defibrillation
Lidocaine Contraindications
Supraventricular rhythms or bradycardias & hypersensitivity
Lidocaine Action
Depresses the automaticity of the Purkinje fibers to raise the depolarization threshold in the ventricular fibers & decrease tendency to fibrillate.
Cardiovascular- decreased conduction rate & force of contraction, prolongs phase 4 depolarization
CNS- tremors, restlessness, clonic convulsions followed by respiratory depression & failure
Lidocaine Side Effects
Sleepy, dizzy, disoriented, convulsions, hypotension, bradycardia, decreased myocardial contraction & increased AV blocks
Lidocaine Adult
1 to 1.5 mg/kg IV/IO, 3 to 5 min
Infusion 1 to 4 mg/min @ 4mg/mL concentration
Endotracheal Admin- 2 to 2.5 mg/kg followed by 10mL flush
Lidocaine PEDI
1 mg/kg IV/IO, repeat @0.5 to 1 mg/kg/min
For ETT use, increase dose by 2 to 3 times
