Leukocytosis Flashcards
4 primary marrow disorders where we see neutrophilia?
CML, other proliferative disorders, sickle cell, neurotrophilic leukemia.
6 common causes of secondary neutrophilia?
Infection, inflammation, stress, smoking, aplenia and meds.
What 4 drugs/meds to remember that will cause neutrophilia?
Lithium, corticosteroids, epinephrine, and GCSF.
What are 3 most common types of infection causing a rise in neutrophils?
Bacterial, pyognenic
Mycotic
Rickettsial
What 2 patient populations to remember that can have a mild increase in neutrophils and why?
Smoking and obese because of an underlying inflammatory state.
What are the 4 different pools of neutrophils and explain demmargination of neutrophils?
Bone marrow, marginated, circulating, and tissue.
Vigorous exercise or stress releases catecholamines and cortisol which basically knock neutrophils attached to the vessel wall into the blood, increasing the neutrophil count.
See this big time in the Pulmonary circulation.
2 big time malignancies causing chronic neutrophilia?
Gastric and bronchogenic
Lets say patient has high neutrophil count, what would make us go straight to bone marrow examination?
If there is leukosrythroblastic smear
What patients are we trying to identify for serious pathology associated with eosinophilia?
Greater than 1500 cells or evidence of end organ disease
What 6 things should be included in the history for a patient with high eosinophils?
B symptoms, rash, diarrhea, allergic symptoms, travel history and food intake.
What kind of infections are most commonly causing high eosinophils and give a specific example and the 4 symptoms you will commonly see?
Parasites
T spiralis from a pig
Eosinophilia, periorbital edema, myositis, and fever
What is the most common cause of eosinophilia in underdevelopinng countries?
Helminth infections
5 allergic conditions leading to high eosinophils?
Allergic rhinitis, atopic dermatitis, angioedema, fungal allergy, and asthma
3 medications that can cause eosinophilia?
Antibiotics, NSAIDS, and antipsychotics
What respiratory tract condition can lead to high Es?
Bronchiectasis
