What is primary amyloidosis? What is secondary amyloidosis?
AL, Ig light chains coming from a clonal B cell or plasma cell disorder associated with lymphoma or myeloma.
AA, acute phase reactant proteins occurring in the setting of chronic inflammation or infection
What tissue is most often biopsied for amyloid?
Abdominal fat

Organs are involved
Kidney then heart (Cardiac arrhythmias), CNS and then obviously several others
Big tongue and racoon eyes
Weight loss and fatigue
2 things found for diagnostic purposes of AL?
Light chains or whole Ig in urine
Increased plasma cells in bone marrow
2 things we can treat the plasma cell burden with in AL?
Melphalen and prednisone/dexamethasone
Can even try stem cell
