What are the indications for early orthodontic intervention?
- posterior & anterior crossbites
- ankylosed teeth
- excessive protrusions & diastemas
- severe anterior and lateral open bites
- ectopic molars
- pseudo class III patients
- cleft palate patients
- severe arch length discrepancies
when will a patient benefit from serial extraction therapy in the mandibular arch?
in mixed dentition analysis of the mandibular arch, if crowding is in excess of 6mm
What is serial extraction?
The orderly removal of deciduous an permanent test determined in a sequence
What are indications for serial extraction:
- Skeletal class I
- Dental class I
- Bimax dental protrusion
- Dental crowding in excess of 6mm
- Light or anterior open bite tendency
- No congenitally missing 2nd bicuspids
Disadvantages of serial extraction:
- increase of overbite
- lingual inclination of incisors
- scarring tissue on extraction spaces
- diastemas
What does the premature extraction of deciduous canines provide?
provides the space for the permanent incisors to assume normal position over supporting bone
What does extraction of the 1st deciduous molars permit?
permits the desired early eruption of the 1st premolars
Why would you extract PERMANENT 1st premolars?
to allow canines to erupt favorably
What is needed for ALTERNATIVE serial extraction?
- borderline malocclusion
- deciduous cuspids present
- moderate loss of arch length
- no irregularity on lower incisors
- no gingival recession
- second deciduous molars in place for a long time
