Lumbopelvic Landmarks

- PSIS

A
Active - Patient does it
Passive - Clinician does it

What to look for in ROM exam:
A
- Quality of motion (ideally smooth, look for areas that are shaky or restricted, quality in end range)
- Compensation of motion (people will try to cheat as they want to perform well in testing, can be a useful clinical indicator)

ROM of Cervical - Active Flexion & Extension
A
- Have the patient seated
- Practitioner may be side on to the patient or behind the patient
- Instruct them to bring their chin to their chest (flexion)
- Instruct them to look at the ceiling as best they can (extension)

ROM of Cervical - Active Lateral Flexion
A
- Patient seated
- Ask patient to bring ear to the shoulder

ROM of Cervical - Active Rotation
A
- Patient seated
- Ask patient to look over shoulder

ROM - Thoracolumbar Flexion & Extension
A
- Patient standing or seated
- Ask patient to lean forward as far as they can, or touch toes if standing (flexion)
- Ask patient to lean back, use thumbs over PSIS as a tactile cue “lean back over my thumbs”

ROM - Thoracolumbar Lateral Flexion
A
- Patient standing or seated
- Ask patient to bend to the side
- ‘Run hand down the side of the leg’ if standing
- Use thumbs on PSIS and hands around pelvis to provide a tactile cue in the direction of lateral flexion.

ROM - Thoracolumbar Rotation
A
- Patient standing or seated
- Ask patient to turn as far as is comfortable
- Instruct patient to return to the center before assessing the other way.

A
1 Spinous space above and just lateral
