Define Medicines Optimisation
“A person-centred approach to safe and effective medicines use, to ensure people obtain the best possible outcomes from their medicines.”
Define Pharmaceutical Care
“The responsible provision of medicines therapy for the purpose of achieving definite outcomes that improve a patient’s quality of life. It is a collaborative process that aims to prevent or identify and solve medicinal product and health-related problems. This is a continuous quality improvement process for the use of medicinal products.”
NZ Health Strategy - Future Direction 2016
Sets the strategic framework for the NZ health system. Outlines some of the challenges and opportunities.
Describes what is wanted “ live well, stay well, get well”. Five strategic themes;
I. People-powered – ‘health smart’
II. Closer to home
III. Value and high performance – best- value for use of resources, equitable health outcomes

People with Long-term Conditions
Prevalence of long-term conditions (LTCs) in NZ is rising & many people have several LTCs.
Supporting people to self-manage is critical with growing burden of LTCs.

Psychological support
Cultural relevance
Systematic follow-up
Draft Pharmacy Action Plan 2015- 2010
Consultation period end of 2015 & expect to be finalised this year.
Describes the future direction of pharmacy services & how they will be delivered in NZ health & disability system.
Better utilised pharmacist expertise in medicine management.

Pharmacy Council of NZ Medicines Management Competence Framework
This framework sets out the medicines management competence standards for NZ.
Outlines four levels of medicines management services
Recognises the relationship between levels of competency required and the complexity of medicines management related issues.
New Zealand National Pharmacist Services Framework
Promotion of optimal medicine-related outcomes from medicines
Encouragement of patient self-management
Meeting the medicine management needs of local populations
and individual patients
Development of medication management services that enhance access, convenience, patient-centred.
Provision of range of clinically effective and cost-effective medication management services.
Alignment of services and integrated into health system.
