What antibiotic would you give to a pregnant woman with a Lower UTI and why?
Nitrofurantoin for 7 days(NOT TRIMETHOPRIM)
Don’t give trimethoprim since its a folate antagonist by inhibiting bacterial dihhydrofolate REDUCTASE)
What class of drug is gentamicin?
Aminoglycoside (protein synthesis inhibitor)
What are the dangerous side effects of gentamicin and is the reason we need to closely monitor patients?
Nephrotoxic
Ototoxic
What is the drug-drug interaction between trimethoprim and methotrexate?
They are both folate antagonists so cause neutropenia due to bone marrow suppression
What is Murphy’s sign?
Palpate RUQ then patient inhales causing pain
Indicates acute cholecystitis
What is the mechanism of action of amoxicillin?
Beta lactam
Cell wall synthesis inhibitor
What is CURB-65?
Confusion
Urea > 7mmol
Resp rate > 30
BP SBP<90 or DBP < 60
Over 65
What drugs are given if CURB65 is above 3?
Co-amoxiclav and Clarithromycin
