Class 3 = K+ blocking drugs (amiodarone)

How many half lives are required for plasma steady state for a drug to be reached?
4-5 half lives
What drug is being prescribed for systemic lupus erythematosus if you test for TPMT actiivty?
Why?
Azathioprine
Azathiproine converted to 6-MP, there converted to TIMP
TPMT is needed to remove the toxic metabolites of 6MP
What is a diuretic agent which is a mineralocorticoid receptor antagonist?
Spironolactone
Also given as last stage for restraint hypertensions
What type of drug is amiloride?
K+ sparing diuretic that acts on ENac channels in the collecting duct
What is the problem with Spironolactone?
Stimulates androgen receptors and oestrogen receptors leading to adverse effects like gynanocmastia
What is a type A adverse drug reaction?
A type of reaction tthat is a dose dependant, frequently predicted reaction
Why is it difficult to administer semiglutide as an oral preparation?
It is a GLP-1 receptor agonist (incretin mimetic) so is a peptide drug so gets degraded in the stomach
What is the loading dose for a drug that needs a plasma conc of 20micrograms/ml with a Vd = 70L?
20micrograms/ml = 20mg/L
20mg/L x 70L = 1400mg
Loading dose is 1400mg
What is the equation for loading dose?
Loading does = steady state conc x Volume of distribution
What is a side effect of a child taking aspirin?
Reye’s syndrome
What is the relationship between rate constant, t1/2, clearance and volume of distribution?
Half life inversely proportional to to Clearacne and proportional to Vd
Has clearance increases, half life decreases
What test should be done before prescribing AMiodarone to a patient with an arrhythmia?
Thyroid function blood test
Since amiodarone contains lots of iodine
What is the mechanism of action of gliclazide?
A sulfonylurea
Stimualtes B cell pancreatic insulin secretion blocking ATP dependant K_ channels leading to lack of insulin release whcih can cause hypoglycaemia
