explain the process of medicines reconciliation
medicines reconciliation is the process to assemble an accurate list of a patients current medication including OTC and complementary medicines.
it requires accurate and reliable sources, recognising any discrepancies and documenting changes, identification of error and past mecial history or drug use
what is pharmaceutical care?
the responsible provision of drug therapy for the purposes of cheivign definite outcomes that improves patients quality of life. - cure of a disease, elimination or reduction of patients symptoms, stopping or slowing disease process, preventing a disease or symptoms
what is the benefit of medicines reconciliation ?
- better communication and record keeping
- redcued waste of time/ effort and less duplication of work
- recued emdicaion error sadn adverse drug errors
- fewer medication related admissions
- fewer issed doses of high risk medicines
what does the process of pharmaceutical care involve ?
pharmaceutical care involves the process through which a pharmacist cooperates with a patient and other professionals in designign, implementation and monitorin a therapeutic plan that will produce specific therapeutic outcomes for the patient.
