What does a SCFE entail?
More common than LCP
-Occurs due to a forced torsion through repeated stress.
-Age: males 10-17 yo, Females: 8-15 yo
-Initial traumatic episode can be as minimal as turning over in bed.
-Acute onset, unable to bear weight
-P! In groin, med. thigh, or knee along with limping and Er of leg while walking
-PROM: on flexion leg ER
-ROM limitation: Flex, ABD, IR
-X-ray diagnosis with seen displacement of femoral epiphysis
**Remember there is a high incidence of avascular necrosis of the femoral head.
What does Legg Calves Perthes disease entail?
Abnormal shape of the femoral head
-Age: 2-13 with average onset at 6 yo
-Initial sign is a limp
-Trendelenburg sign
-Hip ADD and iliopsoas spasms
-Pain referred to the groin, medial thigh, or inner aspect of knee
-ROM limitation: ABD, IR, EXT
-MRI diagnosis with positive bony crescent sign
**Remember there can be an occurrence of avascular necrosis. If this happens they will be NWB and need surgery. once they are out of surgery they will be in a spica cast.
