Laxatives Flashcards
Classify laxatives according to mechanism of action
- Bulk laxatives (dietary fibre)
- stimulant or irritant laxatives
- stool softeners
- 5HT4 agonist
- PG analogue
What is the mechanism of action of bulk forming laxatives
They’re indigestible and they absorb water, swell up and increase the bulk of stool. This causes mechanical distension and stimulates peristalsis and promotes defaecation
Name Bulk forming laxatives
- Agar
- Methylcellulose
- bran
- ispaghula
Bulk forming laxatives should be avoided in which conditions
Magacolon or megarectum
Name stool softeners
- Docusates (docusate salts including diacetyl sodium sulphosuccinate and diacetyl calcium sulphosuccinate)
- Liquid paraffin
What are the adverse effects of liquid paraffin
- Lipid pneumonia if the drug enters the lungs
- Long-term news can cause malabsorption of vitamin A, D, E and K (fat soluble vitamins)
- Leakage of faecal matter through the anal sphincter
Which drug is contraindicated in pregnancy as it may cause reflex stimulation of uterus
Stimulant laxatives
Name the mechanism of action of stimulant/irritant laxatives
They have a direct action on enteric neurons and GI mucosa by increasing prostaglandin and cAMP levels but inhibiting Na+ and K + ATPase activity
Chronic use of stimulus/irritant laxatives leads to what
Atonic colon
Name stimulant/irritant laxatives
- sodium picosulphate
- bisacodyl
- Phenolphthalein
- Anthra quinone derivatives (senna and cascara)
Which drug should be avoided in lactating mothers and why
Anthraquinone Derivatives (senna and cascara)
Because the drug is a secreted in milk
What are the side effects of Anthra quinone
- Skin rashes
- black pigmentation of the colonic mucosa
- discolouration of urine
- can cause colonic atony
Name 5 HT4 agonist
Prucalopride
Name PG analogue used as a laxative
Lubiprostone
Name is osmotic Laxatives
- Magnesium sulphate
- magnesium hydroxide
- sodium sulphate
- sodium phosphate
- lactulose
- polyethylene glycol
