-Hey dr Kim, I noticed you have two secondary lapidus procedures today. Did those patients have recurrence?
Dr. Silkey: Yes both patients had recurrence.
Can I ask what system are you using?
Dr silkey: I’m using Arthrex lapidus system
Ah ok. Have you heard of lapifuse?
Dr Silkey: no I haven’t heard of Lapifuse. What is it
Its the holy grail for bunion surgery. It is a triplanar correction system that corrects deformities caused by bunions.
Its a system providing the instruments to gain that 3 plane correction with implants that allow early weight bearing and a high rate of fusion and correction without having to shorten the first ray of the patient. I guarantee you’ll be out of the room faster with a higher satisfaction rate and zero recurrence.

Dr Silkey: yes tell me more about the system
A system consisting of a Joint prep kit, that is single use and very sharp efficient instruments to prep the joint
It also comes with a fluroscopioc targeting guide for visualization under xray for placement of your interfragmentary screw.
Then it comes with the Triplanar clamp, correcting the deformities amongst all planes easily and then lastly the hardware of a titanium plate- 5 hole or 6 hole, with 2 holes for distal locking, 2 for proximal locking and a compression slot. and your interfragmentary screw and locking or non locking screws.
Because the system is a triplanar correction, it provides the instruments to gain that 3 plane correction in three small incisions. (1) incision medial to the first mtp joint, (2) dorsal medial to the first tmt joint, (3) incision to the webspace between 1st and 2nd metatarsal (release the metatarsal joint) Distractor device with innovative joint prep kit, remove cartilage, put on triplanar clamp , de rotate mtp joint, insert lag screw , secure your plate and screws.
Dr Silkey: Ok what’s the science behind the Triplanar correction?
Bunion operations w wedges. - doing things in one plane didn’t fix the problem
With other companies, you are going to get shortening of the first ray. They will need to taking large wedge resection from the base of the first metatarsal trying to cut off that angle of the first metatarsal cuneiform joint.
Pretty complicated wood working, taking more and more, really short first ray, results in pain and recurrence.
Flat cuts arent necessary. you still get the joint to sit where it should, so you dont have to be using all this extra instrumentation..
Not necessary with Lapifuse, because we havent shortened anything, metatarsal length is exactly the way you want it to be so really all you need to do is rotate and decrease your intermetatarsal angle.
With no flat cuts, you can get 3 planes of correction without shortening.
Dr Silkey: Yea but this seems like a biggg operation, is it difficult? Any down time?
Many bunion procedures require m ore disciplined and rigorous approach. Better to go bigger and get it done, it is weight bearing, back in a shoe in 5-6 weeks and its simple and quick. Our fellowship residents are able to do it quickly. Dr Kim’s resident is able to perform the procedure in under 45 minutes! There little fiddle.
