Lapifuse steps
- Joint prep
Make incision over first Tmt joint, use distractor with (2) 2.5mm pins . Use joint prep kit - Fenestrate using 2.5mm drill and drill guide from prep kit
- Create incision over first mtp joint, insert (1) 1.4mm k wire
- Create incision in first interspace
- Insert clamp over to reduce IM angle
5.use targeting guide to place interfragmentary screw (4.0mm ) from base of first metatarsal, through medial cunueforminto body of intermediate cuneiform . Assemble targeting guide and insert two 1.4mm k wires - Place one kwire through targeting guide to temporarily secure the joints
- Measure: Remove targeting guide and replace with depth gauge to measure
- Pre Drill: 2.7mm drill and guide
- Insert screw ( 4.0mm) (35mm-45mm lengths) with driver
- Select plate: hold plate with temporary pins
- Measure using plate screw depth gauge
- Pre Drill: 2.5mm drill and guide
- Secure plate: with locking or non locking screws (3.5mm) (12mm-40mm lengths)
- (Optional) compression slot : pre drill : 2.5mm drill
- Measure for non locking screw (3.5mm) (12mm-40mm lengths)
- Insert screw: non locking screw into bone
- IF USING 6 HOLE PLATE:
INsert screw: locking or non locking screw through hole (3.5mm, 12-40mm lengths)
