CN 1 Olfactory (+) test =
(+) = anosmia (inability to detect smells)
Cn II visual fields (+) test =
(+) = Monocular visual loss (visual field deficits)
CN II (afferent)/III (efferent) Pupillary response (+) test =
(+) = Absence of pupillary response and/or presence of unequal pupils (anisocoria)
CN III/IV/VI EOMS, Pursuit Eye movements (+) test =
Gaze palsy = (UMN/LMN)
Deconjugate gaze = (UMN/LMN)
(+) = presence of 2 or more saccades, strabismus, impaired eye movements, double vision
Gaze palsy = both eyes limited in same direction
-central or UMN lesions
Deconjugate gaze = one eye or both eyes limited in different directions
-LMN lesion or problem at NMJ
If Trochlear (IV) deficit you will see
- Palsy - downward or torsional movement weakness
- Affected eye will drift up at rest
- Double vision with downward smooth pursuits
- Present at rest with head tilted to one side and chin tucked
If Abducens (VI) deficit you will see
- Palsy - lack of complete lateral eye movement
- At rest, eye will cross inwards toward nose (astropia)
CN V Sensation (motor mastication/sensation face) (+) test =
- motor mastication + test = Weakness/deviation of jaw to ipsilateral side when opened
- sensation face (+) test = Loss of facial sensation of numbness
CN VII Facial Symmetry and Expression (+) test =
If just muscles of lower face (UMN/LMN?)
If muscles of entire face (UMN/LMN?)
(+) test = presence of weakness or paralysis
If just muscles of lower face = UMN
If muscles of entire face = LMN
How to CN VIII 1st Test =
Test for hearing loss with finger rub next to ear

Weber test - lateralization of sound
Normal = hear tuning fork placed on top of head in both ears
Conductive hearing loss = sound heard louder in affected ear
Sensorineaural hearing loss = sound heard louder in unaffected ear

Rinne Test - tuning fork on mastoid process, and then move to ear
Normal = hearing sound longer through air than the bone
Conductive hearing loss = sound heard through bone equal to or longer than air
Sensorineaural hearing loss = unable to hear when tuning fork is on the mastoid
CN IX/X Palatal and Pharynx Control (+) test =
(+) = deviation of palate and uvula to uninvolved side
CN X Gag reflex (+) =
(+) = absence of gag reflex
CN XI Neck motor function (+) test =
(+) = weakness on same side of lesion
CM XII Tongue Movement (+) test =
LMN

(+) LMN = Tongue deviation towards weak side with atrophy or fasciculations
