What are symptoms of anemias?
Pale, weakness, malaise, fatigue, dyspnea upon exertion, koilonychias (spoon nails)
All anemias present with what?
Decreased RBC count, Hb and HCT
What are the characteristics of megaloblastic anemia?
Includes pernicious anemia/ B12 deficiency and folate deficiency MCV increased (macrocytic) RDW increased (macrocytic) MCH and MCHC are variable
What are the characteristics of Hemorrhagic anemia?
MCV, MCH and MCHC are normal (normocytic, normochromic)
What are the characteristics of Iron deficiency anemia?
MCV, MCH and MCHC are decreased (microcytic, hypochromic)
Iron studies- low serum iron, increased TIBC (total iron binding capacity)
What are the characteristics of Sickle cell anemia?
MCV, MCH and MCHC are normal (normo, normo)
Increased reticulocyte count
Blood smear shows sickled cells
What are the characteristics of a bacterial infection?
Increased WBC (total) Increased neutrophils (PMN's) Increased segs (segmented neutrophils, mature) Increased bands or stabs (immature) Increased ESR (sed rate)
What are the characteristics of viral infection?
Increased WBC
Increased lymphocytes
What raises with inflammation?
Increase ESR (sed rate)
What happens to the lipid panel in hyperlipidemia?
Increased total cholesterol
Increased triglycerides (often)
Increased LDL cholesterol
Decreased HDL cholesterol
What are the characteristics of acute hepatitis?
AST and ALT are elevated 10-100x normal
Total bilirubin increased late in disease
Alk phos and GGT increased
What are the characteristics of chronic hepatitis and cirrhosis?
AST and ALT are slightly elevated
Total bilirubin and GGT increased
Albumin decreased in alcoholic liver disease and cirrhosis
What are the characteristics of diabetes?
Hyperglycemia Increased ketones Acidosis (if severe) Glucosurea Ketonurea
What do you find with a urinary tract infection?
Nitrites in urine
Leukocyte esterase
WBC’s and bacteria in urine
What are the characteristics of primary hypothyroidism?
TSH increased
T4 decreased
