Chondromalacia patella
A premature degeneration of softening And roughening of articular surface
Associated with anteversion of the leg, vastus medialis, weakness, trauma, degeneration
Presentations:
RetroPatellar-vague, double clicking, catching sharp pain with crepitus.
Aggravated by sitting walking, running, riding a bike or going upstairs
May cause patellar dislocation
Osgood schlatters
Avulsion of the tibial tuberosity with secondary ossification
More common and athletic adolescent boys, 12 to 18 Yo
Due to micro trauma and stress of the tibial tuberosity
Presentations :
Painful bump, and the tibial tuberosity and is worsen with activity
Maybe mild local, swelling and palpable tenderness, and in severe cases, atrophy and weakness of the quad .
Osteochondritis dissecans
An abnormal ossification in the articulation of the knee predisposed to DJD
presentation:
Necrotic fragment, that may be loose and become a joint mice
Clicking and crepitus
Wilson’s test is used to identify
Meniscal tear
Lateral-hyperextension grinds on lateral mesniscus
Medial- flexion and rotation catch the medial meniscus
Presentation:
Pain
Slow, synovial, swelling
Locking and simultaneously unlocking
Clicking of the knee during activity
May result in DJD
Iliotibial band friction syndrome
Considered a form of tendinitis
ITB tightness may develop a fibrous mass that runs over the femoral condyle attaching to the Gerdy tubercle
Pes anserine bursitis
Lying between the tendons of gracillis , sartorius and semi tendinosis
MC and 50 to 80 year olds
75% of patients with Osteo arthritis of the knee
Overuse/tight hamstrings
Presentations:
Medial knee, joint pain on palpation
Prepatellar bursitis
Located above the patella
Swelling with heat and reduced ROM
MC IN women
Commonly due to compression trauma and kneeling
Patella Plica syndrome
A membrane between the synovium of the patella, and the tibiofemoral joint in many individuals. The tissue is not fully reabsorbed embryologically, and consequently the cavitation of the knee remains in complete.
Presentations:
Medial knee pain
May cause fibrosis of knee causing stiffness or synovitis
Knee gives away and catches
Stabbing px
ACL TEAR
Moi:
Rapid deceleration with valgus and hyperextension
-Swelling within 12h with 75% blood effusion.
-Usually accompanied with MCL tear or med meniscus tear.
