Prepartion of epi and dose in anaphylaxis

A

-IM epinephrine (1 mg/mL preparation)
-Epinephrine 0.01 mg/kg
-For large children (>50 kg), the maximum is 0.5 mg per dose.

A
  • inflammation in the larynx and subglottic airway.
  • RSV, influenze, parainflu
  • Age 6 months to 3 years
  • Fall/early winter

-Inspiratory stridor, Barking cough, Hoarseness

  • If the patient gets worse:
    Inspiratory stridor

    Expiratory stridor (biphasic stridor)

    Stridor at rest

-Best Initial Test * Clinical Dx
CXR not needed-but shows steeple sign

  • Mild (no stridor at rest): humidified air ±
    corticosteroids
  • Moderate/severe (stridor at rest): corticosteroids +
    nebulized epinephrine

Epiglottitis. 3

  • Inflammation of epiglottis and supraglottis
    -s/s High fever, dyspnea, and rapidly progressing obstruction
    Toxic-appearing, difficulty swallowing, drooling, sniffing-position

– Clinical first, laryngoscopy
of cherry-red, swollen epiglottis; x-ray not needed (thumb sign)
