Structures impliquées dans le TOC
DL PFC- leads to deficits in monitoring/working memory and higher level planning (Why can’t I turn off my need to know for sure???)
Supplemental Motor Areas- leads to deficits in inhibitory control over behaviours (Why can’t I stop checking??)
Orbito-frontal –thalamic circuit- leads to deficits in inhibition of irrelevant information, allowing for “concerns about order, danger, violence, hygiene, sex to rivet attention” (“ Why can’t I ignore these thoughts???”)

Thought action fusion (if I think it , it makes it more likely to happen)
Over-importance of thoughts ( if I think a thought, it must be important. I need to pay attention to it)
Intolerance of Uncertainty ( I must know every outcome for sure. To not know something is dangerous)
Emotional reasoning ( if this thought makes me feel like something is dangerous, it must really be dangerous. Emotions never lie)
Inflated sense of responsibility (I must always do everything I can to prevent potential harm).
Adjuvants en TOC outre antipsychotiques
-Agents glutaminergiques: kétamine, mémantine, amantadine
-Agents antiinflammatoires: celecoxib
Approche suggérée pour comorbidité TOC et bipolarité
TCC + stabilisateur (qui n’est pas antipsychotique)
Tx pharmaco de TOC pourra venir déstabiliser le trouble bipolaire. Bien qu’antipsychotiques peuvent être utilisés en adjuvant de TOC, ils ont aussi un risque d’exacerber le TOC
