johnston heart failure part 1 Flashcards
the most common cause of LV systolic dysfunction is from
ischemic heart disease
what stage of heart failure is this
-asymptomatic pt has LVH and or impaired LV function (low EF) previous MI, valvular disease, strucutral heart disease, hemodynamically stable
B
1 year mortality rate is 15-30%
what stage of heart failure is this:
refractory HF, eligible for specialized treatment (mechanical support, transplants)
D
1 yr mortality is 50-60%
what stage of heart failure is this:
-CAD, HT, DM, obesity, metabolic syndrome, excess alcohol, who do not demonstrate strucutral heart disase or symptoms
A
1 yr mortality rate is 5-10%
what stage of heart disease is this:
-patient with current or past symptoms of HF with strucutral heart disease, sob, fatigue, reduced exerise tolerance,
C
1 yr mortality rate is 15-30%
what causes acute heart failure
MI ruptured papillary muscle MR AI toxins
chronic heart failure from
multivalvular disease of dilated cardiomyopathy
progresses edema, weight gain
systolic heart failure features
decreased SV and increased ventricular filling pressure
EF is less than 40%, hypoperfusion
weak, fatigued, reduced exercise tolerance
DOE, orthopnea, PND
diastolic heart failure features
normal ejection fraction
- restrictive/constrictive pericarditis
- hypertensive hypertrophic cardiomyopathy
causes of high output heart failure
hyperthyroidism, anemia, pregnancy, av fistula
high CO but low EF
low output heart failure
ischemic heart disease, hypertension
-dilated cardiomyopathy
PND increases the rate of HF how much
2 fold
S3 gallop increases the rate of HF how much
11 fold
hf and unilateral edema
never unilateral if hf it is always bilateral
if patient is in hf and greater than 65 years old with A fib always check the patients
thyroid
Free T4, TSH
also ABGs
