Q
IV Charting on an I&O Flow Sheet
A
For a patient receiving IV therapy, a nurse must use the I&O flow sheet to document oral intake as well as the amount of IV fluid administered.
When hanging or adding an IV bag or bottle, record the time and the fluid type and amount in the correct columns on the flow sheet.
When changing the IV bag or bottle, or when the fluid has infused, document the actual amount of fluid administered (which is the amount absorbed).
