WHAT IS ISBAR
What is ISBAR used for?
Reporting to the Dr or at hand over to a new nurse. Reports key information about what is happening to the patient
What other chart would you use ISBAR with?
EWS
What does I stand for?
Identity - Introdruce yourself to Dr, tell Dr pationts name dob NHI# what the came in with, current problem eg Vomiting
What does S stand for?
Situation - what the current situation for the client is now eg client is now in isolation
What does B stand for?
Background - Clients medical History eg what relates to now, also what tests and investigations have been carried out
What dodes A stand for?
Assessment - Objective assessment - facts - obs/vital signs what there ews score was - temp pulse resp- pulse ox and bp and Blood gluclose - client is alert/ unconcious/confused/ disoreintated
What does R stand for?
Recommendation - recommend dr comes with in 5 mins
