Goals of admission process:
A
- Collect baseline information in relation to the patient and their health status
- Identifying actual or potential problems
- Validate existing information
- Prepare the patient and family both emotionally and physically for surgery

What information is required to be collected:
A
- Personal and demographic information
- Social, medical, including allergies, medications, existing conditions and those that have occurred in the past which may be relevant to the current issue
- Verbal and written consent for procedures to be carried out
- Information relating to the discharge of the patient

What happens to records:
A
- Stored either electronically or paper based for minimum seven years
- Patients with chronic conditions may have large volumes of old notes which are reviewed during each visit
- Each patient has an MRN which links to electronically stored information across Australia

Definitions of objectives:
A
- An Objective is a short statement about what you want to do, learn, achieve. These are stepping stones to reaching goals.
- A Learning Goal is a broad statement usually non specific and non measurable that suggests your aim for undertaking an activity
- A SMART objective offers structure, focus and a means of measuring successful achievements of your identified learning goal
