95 % - 100% normal
Less than 70 is life threatening

Hypo: <90/60
Hyper: > 140/90

O: onset
P: provocative, what brings its on? What makes it better or worse?
Quality: how does it look, feel or sound? How intense is it

Severity: how bad is it? Rate it
Treatment and timing: what worked in the past for pain? How long did it last?
Understand pts perception: what do u think it means
Values: what level of pain is acceptable to you?
