A

Codeine
- if patient has pain that is not respoved by standard analgesics (paracetamol and ibuprofen)
Midazolam for intravenous sedation
- done on the FP10PCD form
- private controlled drug
Choice of antibiotic not within the DPF (i.e. not able to prescribe on the NHS), with appropriate experience

A

4 main parts

1) patient details
2) what you are prescribing
- Drug
- dose
- form (i.e. tablet, suspentsion..)
- route (oral also known as PO
- frequency (i.e. 4x a day)
- duration (for 5 days)
3) signings
4) practise details
- practise stamp and address
