- examination
- head is <= 1/5th palpable
- vertex presentation
- cervix fully dilated and membranes ruptured
- position of head known
- assess for caput and molding
- adequate pelvis

- analgesia (if midcavity eg pudendal block)
- Urinary catheter (bulb deflated)
