Common cold (nose is blocked)
Tonsillar/adenoidal enlargement
Severe COPD
First trimester of pregnancy
fear of mask/claustrophobia
Pt with limited ability to understand
What are the equipment used to provide IS?
gas cylinders
pressure reducing valves
flow control meter
reservoir bag
gas delivery hoses
nasal hood
waste gas scavenging system
what are some safety features of the equipment used in IS?
oxygen flush button
oxygen monitor
o2 and nitrous oxide pressure dials
one way expiratory valve
reservoir bag
colour coding
what are the advantages of IS?
rapid onset (2-3mins)
rapid peak action (3-5 mins)
flexible duration
rapid recovery
no injection
little side effects
no drugs are metabolised
what are the disadvantages of IS?
equipment expensive
gases expensive
pt must be able to nose breathe
difficult to accurately determine actual dose pt is receiving
what are some signs that pt is adequately sedated?
relaxed & awake
reduced blink rate
mouth open on request
decrease in spontaneous movements
verbal contact maintained
what are some signs/symptoms of over-sedation?
mouth closing - repeatedly
spontaneous mouth breathing
irrational & sluggish responses
decreased cooperation
incoherent speech
uncontrolled laughter
what is the flow rate for inhalation sedation?
5-6L per minute
what can the movements of the reservoir bag tell us?
small movements = check seal & look for mouth breathing
large movements = increase flow rate of gasses
