Inflammatory Joint Disease Flashcards
Immune mediated polyarthritis is usually idiopathic, non-erosive and may be genetic. What do breed is it mostly found in?
Novia Scotia Duck Trolling Retriever
What is the pathophys of immune mediated polyarthritis?
o abnormal B-cell/T-cell reaction with synovial membrane antigens
o Type 1 is idiopathic and is the most common type
o Can be caused by distant infection or drug-induced (ex Doberman with drug induced IMPA to sulfa drugs)
CS of Immune mediated polyarthritis:
Multiple joint pain or swelling– generalized stiffness- shifting leg lameness- pyrexia!!! FEVER
Diagnostic approaches to Immune mediated polyarthritis:
o Arthrocentesis
o Rads- effusion and swelling will be seen, no erosion or bone proliferation
o Hematology, urinalysis
o Synovial fluid – make sure you do AEROBIC and ANAEROBIC cultures—will usually be negative!!!
o Serology
((Overall…. START with rads and bloodwork… and then maybe culture and serology ))
Treatment of Immune mediated polyarthritis:
Abx, immunosuppressive drugs and steroids to put the dog in remission, surgery is uncommon and last ditch effort (Arthrodesis)
Erosive Immune mediated polyarthritis - what is the most impt test?
RADS
Erosive Immune mediated polyarthritis- prognosis
poor prognosis
Tx of patient with Erosive Immune mediated polyarthritis
immunosuppressive drugs as before, gold therapy drugs may be indicated, limited success with arthrodesis because only targets pain, not overall/underlying issue
You see a rad with intracapsular swelling, collapsed jt spaces, subchondral bone erosion and periarticular osteophytosis, what do you suspect?
Erosive Immune mediated polyarthritis
____ progressive polyarthritis and _____ polyarthritis are immune mediated diseases
Feline progressive polyarthritis, greyhound polyarthritis
What is similar to rheumatoid arthritis in dogs?
greyhound polyarthritis
T/F, female cats are most affected to feline polyarthritis and has to be positive forFeLv
False; it is male cats ONLYYY; Doesn’t have to be positive for FeLV, only 66% of them are- its a viral contributor it seems
Characteristics for Erosive polyarthritis in Greyhounds:
- Idiopathic immune mediated polyarthritis, usually symmetrical
- Most common in metacarpophalangeal joints—even in greyhounds that never raced
- Treat with immunosuppressive drugs
Infectious jt disease aka ? is less common than immune mediated arthritis
septic arthritis
what is the most common form of infectious jt disease aka septic arthritis?
Bacterial arthritis (STAPH MOSTLY) next is strep Cats will see Pasteurella and Bacteroides
