What is the pathophys of immune mediated polyarthritis?

A

o abnormal B-cell/T-cell reaction with synovial membrane antigens
o Type 1 is idiopathic and is the most common type
o Can be caused by distant infection or drug-induced (ex Doberman with drug induced IMPA to sulfa drugs)

A

Multiple joint pain or swelling– generalized stiffness- shifting leg lameness- pyrexia!!! FEVER

Diagnostic approaches to Immune mediated polyarthritis:

A

o Arthrocentesis
o Rads- effusion and swelling will be seen, no erosion or bone proliferation
o Hematology, urinalysis
o Synovial fluid – make sure you do AEROBIC and ANAEROBIC cultures—will usually be negative!!!
o Serology
((Overall…. START with rads and bloodwork… and then maybe culture and serology ))

Treatment of Immune mediated polyarthritis:

A

Abx, immunosuppressive drugs and steroids to put the dog in remission, surgery is uncommon and last ditch effort (Arthrodesis)

Need a broad spectrum antibiotics until culture comes back, pain meds, treat MINIMUM OF 4-6 WEEKS, EXTEND AT LEAST 2 WEEKS BEYOND RESOLUTION OF ALL CLINICAL SIGNS to prevent relapse
