Hormonal trigger of ovulation
LH
During normal period, when does progesterone peak?
Following ovulation
Test to confirm ovulation (for couples trying to conceive)
Mid-luteal serum progesterone (>30 nmol/L)
Luteal phase =
Lasts 14 days following ovulation
Commonest cause of ovulatory problems =
Polycystic Ovarian Syndrome (PCOS)
Diagnostic criteria for PCOS
2 of:
1. Oligo/amenorrhoea
2. Polycystic ovaries on scan
3. Clinical/biochemical signs of hyperandrogenism (acne/hirsutism)
Treatment to induce ovulation
- Clomifene citrate/Tamoxifen
- Gonadotrophin injections (recombinant FSH)
- Laparoscopic ovarian diathermy (electrical currents to burn ovary)
Investigation for tubal patency
Hysterosalpingogram
(or diagnostic laparoscopy and hydrotubation (HTB))
When is hysteroscopy used?
Only when endometrial pathology is suspected
What if clomifene doesn’t lead to ovulation?
Take adjuvant metformin
-improves insulin resistance
-reduces androgen production
-improves sensitivity to clomifene
Risks of gonadotrophin injections
Multiple pregnancy, overstimulation
Risks of laparoscopic ovarian diathermy
Ovarian destruction
(however, doesn’t cause multiple pregnancy)
