Define infectious mononucleosis
clinical syndrome caused by primary EBV infection
aetiology of infectious mononucleosis?
close contact (e.g. kissing, sharing eating utensils)
epidemiology?
common in young people ‘kissing disease’
classic triad of symptoms?
Sore throat, Pyrexia and Lymphadenopathy
also general malaise
signs of mono?
- Splenomegaly
- Oedema and erythema of the pharynx
- White/creamy exudate on the tonsils
- Palatal petechiae
- Cervical/generalised lymphadenopathy
- Hepatomegaly
- Jaundice (5-10%)
what will happen if you give someone with mono amoxicillin?
• Widespread maculopapular rash
what will bloods show?
o FBC - leucocytosis
o LFTs - high AST/ALT
what will a blood film show?
lymphocytosis with atypical lymphocytes
gold standard test?
• Heterophil Antibody Test (aka Monospot Test or paul bunnell test
management?
- Bed rest
- Paracetamol and NSAIDs
- Corticosteroids in SEVERE cases
advice?
AVOID antibiotics
avoid contact sports for 4 weeks because of splenic rupture.
