Differentials for pt presenting with strawberry tongue?
Scarlet fever
Kawasaki disease
B12 deficiency anaemia
Organism that causes scarlet fever
Group A Strep - usually Streptococus Pyogenes
Clinical features that form part of the criteria to diagnose Kawasaki disease
Fever for 5 or more days, PLUS 4/5 of the following features:
- Non-exudative conjunctivitis
- Rash (widespread erythematous maculopapular) & desquamation (skin peeling) on the palms and soles.
- Oedema
- Lymphadenopathy
- Mucosal involvement (red mucous membranes) & strawberry tongue
Management of Kawasaki disease
HIGH DOSE ASPIRIN —> to reduce risk of thrombosis
IV IMMUNOGLOBULINS —> to reduce risk of coronary artery aneurysms
Close follow up with echocardiograms to monitor for evidence of coronary artery aneurysms
