Diagnosis of CMV colitis and treatment
Colonoscopy but biopsy. Rx with ganciclovir and ART for relapse. Also require ocular exam
Syphilis with penicillin allergy and pregnant. Treatment?
Skin testing and Penicillin desensitisation. Alternatives don’t work.
Routine prophylaxis for opportunistic infections in HIV
Trimethoprim- sulfamethoxazole for PCP daily for CD <200. Same drug with positive toxoplasmosis serology and CD<100. No other routine prophylaxis.
Clues for legionella. Diagnosis, treatment
Travel history, dirty water
Fever, bradycardia, neuro symps, GI symps, resp symps
Hyponatremia, hepatic dysfunction, hematuria, proteinuria, gram stain no organism
Diagnosis- urinary legionella antigen
Rx- macrolide or FQ
17 yr old girl, mod effusion of a knee. H/o travel
With rash- Lyme/ JIA/ serum sickness
35 yr man from Wisconsin- fever, night sweats, wgt loss, upper lobe consolidation, warty skin lesions, lytic bone lesions in ribs. Diagnosis. Confirmatory? Treatment?
Blastomycosis. Broad based budding yeast from sputum. Treat with itraconazole or amohotericin B
4 yr old, VSD, fever, new murmur, achy legs and hands? Leukocytosis, splenomegaly, RBC casts. Next step? Diagnosis.
Do echo. Infective endocarditis.
