Acute kidney injury after initiation of an antibiotic (cephalosporin, penicillin, ciprofloxacin, trimethoprim–sulfamethoxazole) should raise suspicion for acute interstitial nephritis

How is drug induced acute interstitial nephritis diagnosed?
A
Here, the diagnosis is supported by pyuria and a urine culture without growth. Drug-induced acute interstitial nephritis is classically described as presenting with a triad of an allergic rash, fever, and eosinophilia; pyuria may not always be present. Drug-induced acute interstitial nephritis may present a few days or several weeks after exposure.
