Infections Lecture 6: Renal + GI Infections and management Flashcards
What are the causes of lower urinary tract infection?
Commonly bacteria from E.Coli in the urethra
What are the risk factors of UTI?
- Females
- Post menopause
- Sexual activity
- Pregnancy
What are the symptoms of UTI?
- Polyuria (increased urination)
- Dysuria (pain whilst urination)
- Increased urgency
- Lower abdominal discomfort
- Lower backpain
- Cloudy or bloody urine
- Strong smelling urine
How do you test for UTI?
- Dipstick test
- Urinanalysis
- Culture if required
What are the treatment options for uncomplicated UTI?
- Short course of nitrofurantoin or Trimethoprim for 3 days or 7 days
- Cefalexin (3 days)
- Amoxicillin
- Co-amoxiclav (5 days)
How do you treat uncomplicated UTIs in pregnancy?
- Cephalosporins
- Penicillins
- Nitrofurantoin
- Trimethoprim
What are the symptoms of UTI in children?
- Painful
- Frequent
- Foul smelling
- Cloudy urine
- Poor control over urination
- Back pain and abdominal pain
- Fever and vomiting
What are the treatment options available for UTI in children?
- Nitrofurantoin
- Trimethoprim
- 1st generation cephalosporin
Co-amoxiclav for 7 to 10 days
How do you treat recurring UTI?
- Trimethoprim
- Nitrofuratoin
- Cefalexin (low dose at night)
How does trimethoprim work?
- Broad spectrum antibiotic most UTI causing organisms
- Inhibits folate synthesis
- Rapidly absorbed with high bioavailability
Describe how Nitrofurantoin works?
- Broad spectrum bactericidal
- Only used in UTI infections as it doesn’t penetrate into tissues
- Works by inhibiting cell wall, bacterial proteins and DNA
- Highly soluble in urine (bioavailability increases food)
Describe what complicated UTI is and the treatment options?
- Occurs due to structural abnormality, blockage in urinary system and any other underlaying health conditions
- Treatment: Hospitalisation
- Empirical:
- Fluroquinolone
- Cephalosporin
- IV aminoglycoside
Duration: 7 to 14 days
What are the symptoms of prostatitis (bacterial)?
- Pain problems
- Urinating and ejaculation problems
- Discomfort in the lower back, pelvis and genitals
- With acute prostatitis it’s usually very sudden
What is prostatitis?
Enlargement of the prostate which causes the blockage of the ureter
How do you test for prostatitis?
- Dipstick
- Labtesting urine
- Scans
