A
  1. Infection without abscess
  2. Infection that is not fluctuant
  3. Facial furuncles located within triangle formed by bridge of nose & corners of mouth

What equipment is needed for I and D?

A
  1. Antiseptic solution
  2. Needle & syringe for local anesthesia
  3. 25 – 30 gauge needle
  4. 1% lidocaine
  5. Scalpel #11 blade
  6. Sterile 4x4 gauze
  7. Curved hemostats
  8. Iodoform packing
  9. Sterile cotton swabs

What is the procedure for I and D?

A
  1. Wash your hands
  2. Cleanse area with antiseptic solution
  3. Drape in sterile fashion
  4. Administer anesthetic
  5. Puncture the skin over the fluctuant area
  6. Make a sufficiently wide incision to allow abscess to drain
  7. Probe inside abscess with sterile swab; cultures can be obtained at this time
  8. Express purulent material
  9. Irrigate cavity with sterile normal saline
  10. Pack cavity with iodoform packing strips
  11. Cover with sterile dressing
  12. Arrange appropriate follow-up care

How is the anesthetic administered?

A
  1. Avoid infiltrating abscess cavity
  2. Slowly introduce needle into subdermal tissue and aspirate to be sure you’re not in vessel
  3. Slowly inject anesthetic while withdrawing the needle
  4. Continue to anesthetize the perimeter of the involved area
  5. Wait until area is fully anesthetized

What is the post-procedure care for I and D?

A
  1. Change packing daily
  2. Watch for the following (and report):
    A. Increasing infection
    B. Recollection of pus in abscess
    C. Fever & chills
    D. Increased pain or redness
    E. Red streaks near site
    F. Increased swelling in area
