Assists clinician in recalling important findings, techniques used, or complications encountered during procedure

What is included in a procedure note?
A
- Date and Start Time of Procedure
- Pt name, age, DOB:*
- Procedure performed
- Consent
- Indication
- Contraindications
- Physician/PA
- Anesthesia: (if any)
- Description of Procedure: State how you performed procedure from start to finish (think about what steps you took to accomplish the task) See posted example of procedure note in BB.
- Complications: (includes EBL, side effects, and adverse rxns, if any)
- Instructions and follow up plans:
- Time procedure completed and condition of pt (or Disposition) :
- Sign note with PAS I (now) PAS II (after Aug.)

- Laceration Repair
- Lumbar puncture
- NG tube insertion
- I&D of abscess
- Cryosurgery
- Chest tube placement
- Thoracocentesis
- Intra-articular injections
- Endoscopy
- Colonoscopy
- Removal of any drains or tubes should also have documentation
