Immunohematology Flashcards
Platelets are express and are
express a few antigens and are bathed in donor plasma
Kell, Duff, Kidd and SsU
also implicated in hem trans rxn (esp delayed)
<7
RBC transfuse
> 10 + rapidly declining heme
RBC transfuse
ICU and GI bleeds still have a heme tolerance of
7
Fever + nonhemolytic rxn
normal vs, normal labs
Fever + Hemolytic rxn
LDH, Haptoglobin
Fever + Bacterial contamination
Lactate, blood cultures (consider culturing the donor blood)
Fever + sepsis
see bacterial contamination, VS in septic shock
Allergic Symptoms include…
volume overload = TACO
&
TRALI =abnormal vs and Leukopenia
TRALI
Transfusion-associated Acute Lung Injury (similar to ARDS)
TACO
Transfusion associated circulatory overload
Content of FFP
Contains coagulation factors (as well as all other dissolved proteins, including antibodies)
Does NOT contain cells (with antigens)
NOT immunogenic.
Indications for FFP
Urgent reversal of Warfarin.
Coagulopathy (as in DIC, Hemophilia) with INR>2 OR aPTT > 2x when specific factor concentrates are unavailable.
Platelets have…
Cell fragments (but SOME ABO and HLA antigens present)
They DO NOT contain Rh antigens.
Considered to contain FFP in with the platelets…
When giving platelets to someone…it’s always better to…
administer ABO compatible platelets…
but if that’s not possible, hemolytic rxn with non-ABO compatible pltlts are uncommon but possible
ACS = transfuse if
<8g/dL
Symptomatic and __ - __
7-10g/dL
Cardiac Surgery if
7.5g/dL
Non-cardiac surgery if
<8g/dL
RBC t means the ____ blood cannot contain ANY antigens not present in the _____ body
donor’s……recipient’s
FFP t means the ____ blood cannot contain ANY antigens not present in the _____ body
recipient’s…..donor’s
Platelets contains ______ antigens AND antibodies
ABO
tricky matching, but major rxns are uncommon
