Immune Disease Meds Flashcards
(Glucagon)Classification:
Action:
= Hormone
= Elevates BGL conversion of glycogen to glucose (IM route);Binds to Glucagon receptors & stim/s adenyl clyclase, which increases cAMP (IV route), increase in cAMP causes an up-reg/ of Ca channels in SA & AV nodal cells
(Glucagon) Indications:
Contraindications: Known hypersensitivity, Bradycardia from other etiology
Adverse Effects: Anxiety, Chest palp/s, H/A, N/V, Increased blood sugar (hyperglycemia)
Adult Dose for Hypoglycemia: 1 mg IM/IN
Pediatric Dose for Hypoglycemia:
<20 kg 0.5 mg IM/IN
>20 kg 1 mg IM/IN
= Hypoglycemic & IV/IO access isn’t obtainable, Bradycardia suspected from Beta-Blocker or Ca Channel Blocker OD.
= hypersensitivity, Bradycardia from other etiology
(Glucagon) effects:
Doses:
= Anxiety, Chest palp/s, H/A, N/V, Raised BGL (hyperglycemia)
= IM/IN <20kg 0.5mg >20 kg 1mg IM/IN
(Dextrose) Pre/rq for IVP:
Pharmacodynamics: Water-soluble monosaccharide
= syringe saline pull for blood & push 5x
= Water-soluble monosaccharide
(Dextrose) ind:
Contraindications:
= Correct/ing hypoglycemia
= Known hyperglycemia
(Dextrose) Effects:
Adult Dose:
Pediatric Dose:
= Tissue necrosis w/ extravasation, May worsen hyperglycemia
= 25Gs IV/IO D10% in 50 mL (5 gram) IV bolus (max 250mL), 25 Gs IV/IO 10%DEX in 50mL (5Gs) IV bolus (max 250 mL)
= 5-10 mL’s/kg of D10
(Diphenhydramine) never use for:
dynamics:
= asthma b/c makes tired)
= H1/ 2 histamine blocker
(Diphenhydramine) Inds:
Contraindications:
= Allergic reaction, Anaphylaxis,Extrapyramidal reaction, Sedation
= Known hypersensitivity to med/
(Diphenhydramine) Effects:
Adult Dose:
Pediatric Dose:
= Drowsy, Dizzy, coordination loss, Blurred vision, Hypotension, Tachycardia
= 25 – 50 mg slow IV/IO &IM
= 1-2 mg/kg slow IV/IO or IM (Max dose: 50 mg)
(Solu-Medrol) Dynamics : Indications:
Contraindications:
= Synthetic glucocorticoid Anti-inflammatory; suppress immune response
(same as Dexy)
= Bronchial asthma, COPD, Anaphylaxis
= hypersensitivity to med/
(Solu-Medrol) Effects:
Adult Dose:
Pediatric Dose:
= Increase BGL, delayed effect (usually not seen)
= 125-250 mg IV/IO
= 2 mg/kg IV/IO (max 60mg)
(Dexamethasone)dynamics:
Indications:
Contraindications:
= Synthetic glucocorticoid Anti-inflammatory; suppress immune-response
= Bronchial asthma, COPD, Anaphylaxis
= hypersensitivity, (Relative) Known fungal infection
(Dexamethasone)Effects:
Adult Dose:
Pediatric Dose:
= Retains fluid (Peri/edema) & May delay wound healing & small Increase in BGL (careful w/ diabetics)
= 10 mg IV/PO “better>sol-med”
= 0.5 mg/kg (max 10 mg IV/PO)
(Mag-Sulfate) class:
Dynamics:
Indications: Bronchial asthma, Torsades des pointes, Eclampsia
Contraindications : High-degree of Heart blocks (AV node), Shock, Dialysis, Hypocalcemia, V-Fib/pulseless, V-Tach
Adverse Effects: Flushing, Sweating, Bradycardia, Resp/ depress/ , Hypothermia (need to shiver <86F)
Adult Dose for Bronchodilation: 1-2 grams IV/ 10-20 mins (Infusion)
Pediatric Dose for Bronchodilation: 25-50 mg/kg IV/IO (max 2G) over 15-30 minutes (Infusion)
=Organic-salt Ca-blocker “between handgun & bazooka”
= very powerful nonadrenergic bronchodilator: act as physiologic Ca channel blocker for cases of severe (phase 2) asthma not responding to traditional adrenergic med/s, Smooth M.s relaxer/dilator
(Mag-Sulfate) Indications:
Contraindications:
Adverse Effects: Flushing, Sweating, Bradycardia, Resp/ depress/ , Hypothermia (need to shiver <86F)
Adult Dose for Bronchodilation: 1-2 grams IV/ 10-20 mins (Infusion)
Pediatric Dose for Bronchodilation: 25-50 mg/kg IV/IO (max 2G) over 15-30 minutes (Infusion)
= Bronchial asthma, Torsades des pointes, Eclampsia
= High-degree of Heart blocks (AV node), Shock, Dialysis, Hypocalcemia, V-Fib/pulseless, V-Tach
